Workshop 2008: registration

NOTE: PLEASE READ THESE DIRECTIONS. On the PayPal payment form, you must:
• enter your full name
• any applicable degree after your last name (i.e. MD, RN, MFCC, PhD, etc. - example: Jane Doe, MFT)
• INCLUDE YOUR MAILING ADDRESS in the "shipping address" section so that we can send you a workshop registration packet plus set up your CME or BRN/CEU credits if applicable.

Please don't register more than one person at a time - we won't know who else you are trying to register. If you register for someone else (and are paying with your credit card), you will need to send an email to workshop@rachelremen.com immediately with complete information about the person that you are registering for (otherwise, YOU will get their materials, and YOU will be registered for the workshop).
